Personal Tax Documents
Identification & Basic Info:
- Driver’s license or government-issued ID.
- Social Security numbers (for taxpayer, spouse, and dependents).
- Dependent proof (Birth certificate, SSNs for dependents. marriage certificates, letter from an authorized adoption agency or from the authorized placement agency or applicable court documents that verify your relationship to the child).
Forms Deductions & Credits:
- Supply all W-2 forms from all employers.
- Interest and dividends (1099-INT)
- Dividends (1099-DIV).
- Self-employment income (1099-NEC or 1099-MISC).
- Unemployment compensation (1099-G).
- Social Security benefits (SSA-1099).
- Retirement distributions (1099-R).
- Capital gains (1099-B).
- Any other income documentation (side gigs, crypto, etc.).
- Documentation for any retirement contributions (IRA, 401(k), etc.).
- 1098-T (education expenses).
- 1095-A (health insurance marketplace statement).
- Childcare expenses (Proof of childcare expenses, with provider’s name, address, and EIN/SSN).
- Medical expenses (if significant).
- Energy-efficient home improvement receipts (if applicable).
- Charitable donation receipts.
- Student loan interest (Form 1098-E).
- Proof of education expenses (tuition, fees, 1098-T form for students).
- Mortgage interest statements (Form 1098).
- Property tax statements (for possible credits).
K-1 forms:
- For any partnerships, trusts, or S-Corporation income.
- Documentation of any alimony received (pre-2019 divorces).
Schedule C Forms:
- To report income or loss from a business you operated or a profession you practiced as a sole proprietor.
Tax Payments & Withholding:
- Proof of any estimated tax payments (if made during the year).
